A death needs to be registered within five days at the registry office in the sub-district of the council where the death occurred.

What you'll need
  • The doctor's certificate.
  • The deceased's medical card, birth certificate and if applicable any marriage certificates.
If you can't find these, you should contact the registrar.

What you'll be asked about the deceased
  • The date and place of birth and death.
  • Their address.
  • Their full name and maiden name (if applicable).
  • Their occupation.
  • Details of any pensions and allowances they were receiving.
What you'll be given

  • Certificate of burial or cremation - for you to give to the funeral director. This is also known as the 'green form'.
  • Certificate of registration of death - if the deceased was receiving a state pension or other benefits, you may need to send this to your local social security office.
  • Death certificate - a small charge will be made for this. Many organisations (like banks and building societies) may ask to see an original copy of this, so it might be a good idea to ask for several certified copies. Usually at Egg we don't need an original copy, although occasionally the insurers who provide Egg Insurance policies do require an original death certificate.
